This week marks a significant milestone as we celebrate the graduation of our third round of Phase III Cardiac Rehabilitation Maintenance Program (CRMP) participants! From day 1 to completion, witnessing the remarkable growth and resilience of our participants has been nothing short of inspiring. The Phase III CRMP plays a crucial role in the continuation of care after Phase II Cardiac Rehabilitation, offering guidance on exercising independently and confidently post-cardiac event. In this program, we empower individuals to embrace a proactive approach to their health by teaching them not just what to exercise but also how, fostering a sense of self-efficacy and independence. Our aim is to eliminate the need for constant supervision, allowing participants to navigate their fitness journey with confidence.Beyond graduation, we don't just say goodbye—we stay connected and committed. By keeping in touch, we ensure ongoing support, accountability, and encouragement for our participants well past Phase III. It's not just a program; it's a community dedicated to long-term cardiovascular health. The impact of this program extends far beyond graduation day, and we're proud to be part of each participant's journey towards a heart-healthy life. 💙To the Green Bay Packer's Foundation, we extend our heartfelt gratitude for making a difference in the lives of countless individuals. Your support fuels our passion for creating a healthier and stronger community.Spread the word! Share this post with anyone who might benefit from our FREE 2024 Phase III CRMP. Let's continue working together towards a future with fewer cardiovascular events and healthier hearts. 💙🤝#CardiacRehabilitation #HeartHealth #GraduationWeek #WellnessJourney #CardiovascularHealth #CommunitySupport

Happy Friday! – ANNOUNCING A NEW AWS-PATIENTSOURCE CLIENT 🚨 HealthBus Trust Dorset are at the cutting edge of early intervention primary care, to the degree that they are working closely with (99+) James Kingsland OBE who is a renowned lead on addressing health inequalities. Creating a ‘national learning network’ on health inequalities. Their work has also attracted the attention of NHS England Medical Director, NHS England Professor Stephen H Powis who toured the bus and had lunch with the service users recently. The HealthBus team has pioneered mobile primary care with their CQC registered and compliant clinical bus, which is fitted out to the same exacting standards as a brick and mortar GP surgery. Now a client of AWS-PatientSource, sim enabled IPads and the accessibility of the Amazon Web Services (AWS) cloud enables care to be taken where it is needed most. Did you know that someone who is homeless is 40 x less likely to visit a GP when ill? This leads to the life expectancy of those on the streets being 40 years less than the average across Dorset. The HealthBus is doing everything they can to fight these inequalities, and we are proud that PatientSource is the solution supporting their efforts. Underfunding similar to education trends. Thousands of people are estimated to have died in the last year because of overwhelmed ambulance and emergency services. There are 7.2 million people in England, more than 10 percent of the population, on waiting lists for treatments like hip or knee replacements, back surgery or cataract operations. And hundreds of thousands of people have had a doctor’s referral for outpatient care at a hospital rejected because there are no available appointments — they are simply bounced back to the doctor to begin the process again. There was a 35 percent increase in people choosing to self-fund care between 2019 and 2021, with “market-beating growth” reported in the self-pay market since the Covid pandemic. The number of people self-funding operations like hip and knee replacements more than doubled. And there are indications that some of the people paying for private treatment are doing so not because they can afford to, but because they can’t afford to wait: Between 2019 and 2021, as self-funding increased, the number of people paying with private insurance decreased; people are reportedly taking out loans to pay for operations and, in a development that will be familiar to Americans but is something quite new in Britain, more and more people are turning to GoFundMe to raise money for medical treatment.#access #demography #publichealth #publicprivatepartnerships

As a former employee of Howard County General Hospital and a former county government employee, I had been involved in many discussions about our local hospital; the services it provides; the value of the system it belongs to and the needs of the community.Howard County has ONE hospital. The emergency department is the "front door" that many experience. Through that door comes patients suffering from strokes, heart attacks, mental health crises, overdoses, broken bones, COVID, infections and more. The goal of everyone - from medical staff to state and local officials - is to provide everyone in our community with the right level of care, as quickly and efficiently as possible. Does a name change help in that effort? In a way, it does. It highlights the integration into the Hopkins system; it reinforces the fact that this is a non-profit institution, not one run by government; and it better explains the scope of services. There is symbolism too. But it's a purposeful step in the right direction.https://lnkd.in/eJNQbz75 #experience #mentalhealth #help #change #hospital #covid #community

Celebrating The Power of GivingGiving Tuesday lasts all week as we focus on how each and every single act of generosity offers light for those in darkness. At Adventist Health Glendale’s Center for Behavioral Health and Recovery, patients receive compassionate and whole-person services and care. Yet, with continuous occupants it’s often challenging for staff to take the time to upgrade and update beyond the Center’s basic needs. Imagine what a beautiful mural or colorful artwork can do to elevate your mood, and how it can assist the therapy of a patient often in the Center for days or weeks. Every contribution can help improve the healing environment with the smallest comforts. Show your care for others today!Contributions will go towards the much-needed larger projects of:Medical beds ($8K per bed);Mural/painted walls in unit; New furniture for common room/craft roomLandscaping on patio;Outside chairs and sunshades for patients to enjoy the patio;Chain link fence outside outpatient area;Lower area basketball court needs resurfacing – beneficial for acute and outpatientTransportation to and from outpatient program (would help to grow the outpatient program)With your help, together we can make each day better and brighter.https://lnkd.in/dRBp4NBU

With Thanksgiving tomorrow, and the winter holidays coming up, families around the country want to travel. That includes families with children or adults -- including children in adult bodies -- with special health needs, serious illnesses and disabilities. Medicine wants to help these families enjoy every minute, live their lives, see the world (not to mention coming back and going to school). But these families face a fear that many others cannot appreciate: "What if something goes wrong, and the medical information for my child is at home?" "What if we get into a car crash and I'm passed out, so I can't tell the Responders what my child's needs are?" Such concerns are not just barriers to care -- they become barriers to experiencing life to the fullest. Communities of care across the United States, from Oregon to Virginia, and coming soon to several other states, are working to change this dynamic by making critical information about Children & Youth with Special Health Needs portable -- AND ACCESSIBLE in real-time, at the patient, no matter where they're from. (Ask yourself if the same approach would be helpful to families who are crossing borders to safety from a war zone? YES!)That's why we're proud to partner and power regional initiatives like KIDS OF VALR -- the #Virginia Life Registry -- and the HERO Kids Registry in Oregon (via the Oregon Center for Children & Youth with Special Health Needs). Whether families are traveling to Virginia Beach, or to Washington, D.C., or to Colonial Williamsburg -- or to Portland, New York, Philadelphia, San Francisco, Dallas...and anywhere in between -- their critical data travel, too. Patients, families, school nurses, and clinicians both in the field and at the hospital can breathe a little easier. This holiday season, consider signing your child up for KIDS OF VALR in Virginia or the HERO Kids Registry in Oregon so clinicians across the state can offer maximally informed emergency care. ADVOCACY AT THE HIGHEST LEVEL: "Martin Luther King Jr. Community Hospital forms the cornerstone of Los Angeles County’s public investment in the health and well-being of millions of residents who have few other options for medical treatment. The private facility has been a remarkable success since it opened 8½ years ago. Now, the new hospital is also in jeopardy, not because of the quality of medical care or management, both of which have received stellar reviews. The problem is a funding structure that relies on supplemental payments from the state and the county but does not take inflation into account. The hospital is also a victim of its own irreplaceable role in the community. So many South L.A. County residents are uninsured or underinsured and therefore do not visit outpatient clinics — few of which exist in that underserved part of the county anyway — that their medical conditions worsen until they have to come to the hospital...."These and other services represent the sort of investment in public health, mental health and medical care that county government has made the foundation of its “care first” mission in the last decade. The state ought to back these investments with commitments of its own...."There are many competing interests vying for a piece of the state’s tight budget, but essential medical services for millions of Southern Californians should be at or near the top of the list."https://lnkd.in/gJRpz2eX
